INQUIRY 16-03, IRS Form 1095-C

Summary

Based on Public Law 111-148, the National Finance Center (NFC) will issue a new Internal Revenue Service (IRS) form, Form 1095-C, Employer-Provided Health Insurance Offer and Coverage Insurance. Form 1095-C will be distributed to the applicable employees and transmitted to the IRS.

New Option, Change Paper W-2 & 1095C, added under Preferences to elect a no mailing option and allow employees to have their IRS Form 1095-C available via EPP only.

New report, 1095-C Statement, added to the Financial Reports menu.

Implementations

IRS Form 1095-C

NFC will produce and distribute IRS Form 1095-C to applicable individual employees via mail or EPP. All employees classified as full time (an employee who works 130 hours in any calendar month) will receive IRS Form 1095-C.

Reporting Center

Agency users who do not have access to the Reporting Center (or the 1095-C Statement report) should do so via their Agency Security Officer.

Employees who are not current EPP users (or need a new user identification (ID) or password) can locate sign-up instructions under My EPP. Log on to the Internet and go to the NFC Web site (www.nfc.usda.gov). Select the HR and Payroll Clients page from the MyNFC drop-down menu and click the My EPP icon on the HR/Payroll Application Launchpad.

The procedure manuals for these applications, which are available online at the NFC Web site, are being updated to include the information in this bulletin. To view and/or print these procedures, select HR and Payroll Clients from the My NFC drop-down menu on the NFC homepage. At the HR and Payroll Client page, click the Publications tab in the center ribbon and select the applicable category to access the procedure manual within the category.
